Forgotten souls? How to get them?

I am paragon 164 rogue and cannot for the life of me find any forgotten souls! I have tried everything! Tree of whispers, hell tides, world bosses, nightmare dungeons, you name it. Any suggestions would be helpful. I am playing on hard level. Thanks!
